What to Bring to Your Appointment

  1. Your best glasses.
  2. Your insurance card.
  3. Your referral from your primary care physician or ophthalmologist.
  4. Please make a list of all your medications, inhalers, atomizers and vitamins, including dosages, that you are currently taking.
  5. Please make a list of all surgeries that you have had.
  6. Please make your post-appointment transportation arrangements prior to your visit (depending on any specialized testing or treatment that may be done, your visit may last several hours).
